The dive shop needs a doctor’s signature. We are the doctor.
Checked yes on the diver medical questionnaire? A board-certified physician reviews your records, sees you on a short video visit, and signs your RSTC medical clearance the same day when it is medically appropriate. $91 flat. Full refund if we refer you instead.

The RSTC Diver Medical is a niche form. Most primary care physicians see it once a decade, and many decline to sign out of unfamiliarity or liability concern. Meanwhile your course starts Saturday and the shop cannot legally train you without it.
Dive medical clearance is a real medical decision. It deserves a physician who has read the form, knows the screening system behind it, and will actually evaluate you rather than turn you away.
PADI standards make the physician’s call binary: approved or not approved, no conditions attached. We take that seriously. Every clearance here requires your relevant records plus a live video visit. No clearance is ever issued from a questionnaire alone.
If your history cannot be safely cleared over video, we say so, point you to an in-person dive medicine evaluation, and refund every dollar.
Complete the intake with the questions you flagged on the diver medical questionnaire and upload the relevant records. Recent spirometry for asthma, for example.
Pick a time that works. You meet Dr. Kawalek on a scheduled video visit of about 15 minutes. He asks what a dive physician needs to know, not a script.
When clearance is appropriate, the completed RSTC Physician’s Evaluation Form arrives in your inbox signed with his name, NPI and state license number. Show it to the shop and go dive.
An honest dive medical service has to be able to say no. Here is roughly where the line sits. The physician’s judgment on your actual records decides every case.
$91 flat. Records review, live video visit, signed RSTC form. Full refund if we refer you to in-person care instead.

It is the standard Diver Medical Participant Questionnaire used by PADI, SSI, NAUI and most training agencies worldwide. If you answer yes to any screening question, the dive shop cannot train you until a physician completes and signs the Physician’s Evaluation Form. That signed form is what DiveSlip provides.
Very common. Most primary care physicians rarely see the form, and many decline to sign it out of unfamiliarity or liability concern. Dive medicine is a niche. DiveSlip exists because divers with manageable medical histories were being turned away or left waiting weeks before a booked trip.
Yes. PADI standards do not require a specific format for the physician evaluation. They require a licensed physician to evaluate you and sign the form. Your evaluation here is a live video visit with a board-certified physician licensed in your state, followed by the completed, signed RSTC form. It is not a questionnaire rubber stamp.
Sometimes. Mild, well-controlled asthma with normal recent spirometry and no exercise or cold triggers can be compatible with recreational diving. That determination requires your records plus the video visit. If your history does not support a safe clearance online, we refer you to an in-person dive medicine evaluation and refund you in full.
Evaluations are live video visits at scheduled times, most days of the week. When the physician approves your clearance, the signed form is emailed the same day. Most divers complete the whole process within one to two days of booking.
Training agencies generally treat physician clearance as valid for 12 months. Dive professionals and divers with flagged medical history typically renew annually. Keep a copy with your certification card.
A flat $91. That covers the records review, the live video visit and the signed RSTC Physician’s Evaluation Form. If the physician cannot safely clear you online and refers you to an in-person evaluation instead, you receive a 100% refund.
