What we collect

Your identity and contact details, your answers to the diver medical questionnaire, any records you share for the evaluation, your visit booking, and payment confirmation from our payment processor. Health information you provide is protected health information and is handled under HIPAA.

How we use it

To evaluate you, conduct your visit, issue your signed evaluation, support you, and meet our legal and record-keeping obligations as a medical practice. We do not sell your information. We do not send your identity or health information to advertising platforms, and no advertising trackers load on our intake or booking pages.

Who processes it for us

Trusted service providers under appropriate agreements: payment processing (Stripe, which handles your card and gives us only payment status), email delivery, secure data hosting, and the video platform used for your visit. Each receives only what its role requires.

Verification

Your signed evaluation carries a verification code. Anyone with the code, such as a dive shop you hand the document to, can confirm the document is genuine and current. The verification page shows validity, not your health information.

Your choices

You can request a copy of your records, correction, or deletion where the law allows by emailing support@sickslip.co. Medical practices must retain some records for legally mandated periods.
